REPUBLIC OF PERILO MINISTRY OF EXTREME SITUATIONS

SOP 4: STAFF DIRECTIVE

1.  Purpose:  This standard operating procedure establishes a standard form for communication, coordination, and documentation of actions in the National Emergency Operations Center.

2.  Application: This form will be used (1) by the staff of the National Emergency Operations Center, and (2) by the staffs of Provincial and local Emergency Operations Centers to record information received in the emergency operations center, assign it for action, record actions taken, and to route the form to other interested sections.

3.  Preparation: The Staff Directive form will be completed according to the following guidelines:

a.  Date is the date of the action or information that causes the form to be initiated.

b.  Time is the time the form is initiated.

c.  Event is the event name or other designator.

d.  Priority is assigned as 1 (highest - immediate action required), 2 (medium - action may be deferred but should be taken within this duty shift), or 3 (low - action should be taken within 24 hours)

e.  Action No. is a number assigned sequentially as information, mission taskings, or reports are received.

f.  Source and Contact are the source of the information and contact information for that source.

g.  Life Hazard provides a short description of any hazards to human or animal life as a result of the incident.

h.  Check whether the information is for Action, for Information on actions being taken by other organizations, or is for Reference.

i.  Situation/Event/Information is used to record the information received.

j.  Actions Required identifies the first assessment of the actions that the eemergency operations center staff should take based on the information received.

k.  Staff Sections Assigned And Actions identifies the key staff sections interested in this form and what actions they should take.

l.  At the bottom of the form space is provided for Other Additional Information.

m.  In the subsequent number sections of the form staff officers working action items indicate their Section, the Time action was taken, and By whom the action was taken.  Space is provided to describe Additional Information developed, Actions Taken, and Further Action that may be required.

 

n.  The column on the right hand side of the form is used to identify sections that need to see the form by circling the appropriate name.  The staff officer handling the form in each section adds his or her initials to indicate the form has been seen.

4.  Routing:  A staff directive form is completed by the individual first receiving the tasking, information, report, etc.  That individual should make a determination of at least the first section that should take action on the form's content and route the form to that section.  Each subsequent section should complete a section of the form recording actions they take, reports received, etc.  Additional forms to record actions should be numbered with the same sequential number and stapled or clipped to the first form.
