SOP 5: QUICK LOOK DISASTER REPORT

1.  Purpose:  This standard operating procedure establishes a standard report for use by local jurisdictions in rapid reporting of disaster impacts to the Provincial and National Emergency Operations Center.

2.  Application: This form will be used by the staff of all Emergency Operations Centers and Civil Defense Offices, including other government agencies, Provinces, and Cities, Towns, Villages, Townships, and Recognized Settlements, in rapid reporting of disaster impacts to the National Emergency Operations Center.

3.  Preparation: The Quick Look Disaster Report form will be completed according to the following guidelines:

a.  From and Date-Time Group will identify the reporting jurisdiction and the time of the data in the report.

b.  Part A lines 1 through 5 are  answered Yes or No. When an emergency operations center is opened, the hours of operations are reported.  For shelters, the number opened and the current shelter population is reported. 

c.  Part B line 6 is the date-time of onset of disaster impacts.  Line 7 is a one or two word description of the type of incident.  Lines 8 and 9 provide the number of dead and injured.  Lines 10 through 13 are color coded: BLACK - no capability or capability effectively destroyed; RED - very limited capability, heavy damage; YELLOW - significant damage but still able to provide some level of service; GREEN - operational with minimal impacts.

d.  Part C line A provides the percentage of the jurisdiction's available resources that have been committed to response work.  Lines B and C report the presence of outside assistance.  Line D identifies the key resource need the jurisdiction has at the time of the report.

4.  When Reported:  A Quick Look Disaster report should be submitted as soon as it is possible for a jurisdiction to complete an initial assessment of the disaster's impact and the response.  This should normally be within the first 1 to 8 hours after impact. 

5.  Transmission: 

a.  Reports may be transmitted by any available communications means.

b.  When transmitting the report by voice (by telephone, cellular telephone, satellite telephone, radio, or internet telephone call) read only the line number and the text of the actual report.  Do not read the text that identifies the material on the line.
